At Grosvenor Street Physiotherapy Limited, we are committed to maintaining the trust and confidence of our clients. We want you to know that we are not in the business of selling, renting or trading email lists with other companies and businesses for marketing purposes. Our Privacy Policy details when and why we collect your personal information, how we use it, the limited conditions under which we may disclose it to others and how we keep it secure.

What information do we collect about you?

We collect information about you when you complete a consent form, we use this information to create a patient record on our Practice Management system. We also collect information when you voluntarily complete customer surveys, provide feedback and participate in competitions. Website usage information is collected using cookies.

Consent Forms

When you arrive at our clinic as a new patient, if you have changed your personal details or if we have changed our terms and conditions we will ask you to complete a Patient Consent form, this form requests the following information:

  • Name
  • Address
  • Date of Birth
  • Telephone number
  • Email address
  • Your GP and their address
  • Any current medication that you are taking
  • Health Insurance Provider details
  • The legal data retention period for physiotherapy is 8 years.

How will we use the information about you?

We collect information about you to create a patient record where all of your medical documents that you have provided, treatment notes and forms that you have completed are stored securely.

We will also use your details to send you appointment reminders, appointment information, newsletters and information about other services that we offer that we think may be of interest to you.

We will never pass on your details to a third party without your written consent. We will never share your information for marketing purposes with any companies outside of Grosvenor Street Physiotherapy Ltd.

Consent

In some instances we may have to write to your GP, Consultant, Health Care Provider, Employer, Occupational Health Source or solicitor. On our consent form there is a consent section that we will ask you to sign. We will not send any information unless we receive a signed consent form the body that has referred you for treatment and have your signed consent on our registration form. Please refer to your referring bodies for their privacy notices.

Email Appointment Reminders

We send appointment reminders by text to all of our patients that have provided us with a phone number.

Marketing & Mailing Lists

We would like to send newsletters, offers and information about our services that maybe of interest to you. If you have consented to receive marketing, you may opt out at a later date. You have a right at any time to stop us from contacting you for marketing purposes.

We use Campaign Monitor, this is a third party provider that we use to deliver our e-newsletters and offers. The only information that we enter on our Campaign Monitor database is your email address and your name. We gather statistics around email opening and clicks using industry standard technologies to help us monitor and improve our e-newsletter. You can opt out by clicking on the link on the email that you have received and following the instructions. Please note that if you opt out of this mailing list you will still receive appointment reminders by email.

Access to your personal information

You are entitled to view, amend or delete the personal information that we hold. If you would like a copy of some or all of your personal information, please send your request to our Data Protection Officer (Practice Manager) Louise Topley.

Website Cookies

Our websites use cookies to collect information. This includes information about browsing behaviour by people who access our websites. This includes information about pages viewed and the customer journey around our websites. Detailed information is set out in our Cookie Policy.

Google Analytics

When you visit www.gsphysio.co.uk we use Google Analytics to collect standard internet log information and details of visitor behaviour patterns. We do this to find out how many visitors we have to various parts of the site. This information is processed in a way that does not identify anyone. We do not make and do not allow Google to make any attempt to find out the identities of those visiting our website.
